Current district policy is that all medications must be brought to or picked up from the health office by a parent or guardian. Please DO NOT send in medications with children
When it is absolutely medically necessary that a medication be administered during school hours, a written medical order is required from a physician
The order must specifically state:
child's name
medication
dosage (such as 'one tablet (or tsp) every 4 hours')
time to be given (if necessary)
any pertinent instructions.
Please use the proper form (see below to download forms)
Sign the parental consent/authorization
All medications must be given to the health office in the original container with the pharmacy label.
The label must match exactly the medication provided.
Over-the-counter medications (including cough drops and Benadryl) follow the same procedures (although a pharmacy label is not required).
Please check expiration dates on all medications submitted. Medications that have expired will not be accepted.
If possible, send in medication that will not expire before the end of the school year.
