Email your school counselor or stop by the School Counseling office to complete an appointment slip. 

SENIORS: Your School Counselor cannot send your transcript to ANY college, university, trade school, military branch or employer if this form is not submitted. Students under the age of 18 must have this form signed by a parent/guardian. Students 18 and older may sign the form themselves.

Your School Counselor cannot send your transcript to The National Collegiate Athletic Association (NCAA) if this form is not submitted. Students under the age of 18 must have this form signed by a parent/guardian. Students 18 and older may sign the form themselves.

This packet includes a Student Self-Assessment, Parent Brag Sheet, Sample Résumé and a Transcript Release Form. All four must be completed and submitted to your School Counselor TOGETHER. Packets must be completed at least TWO WEEKS prior to your EARLIEST college application deadline.

You may be permitted to take an 8th class instead of lunch. Be advised that the course you select may not run 5th or 6th period, but the course will replace your lunch option. You may elect to take the course either Pass/Fail or for a letter grade. Only one course may be selected as a Pass/Fail option. ONLY courses that are listed on the contract are eligible to replace lunch.

Beginning June 1, 2023, the new process is online and streamlined, and can be accessed at MyWorkingPapers.nj.gov. 

Students should no longer visit the HS main office or guidance office to request or process working papers.
